Gynecomasty is a surgical procedure that consists of removing the excess gland of a male patient who has suffered an increase in his breasts. The techniques used to correct gynecomasty are different in each case as the causes of gynecomasty are also different. It may happen that the patient has had a small increase in the gland derived from the use of anabolic substances or due to pubertal development, in this case we will proceed to remove the excess gland through an incision around the areola at the lower edge which will not leave a noticeable sign in the long term. This type of intervention should always be completed with a vibramatic liposuction to regularise the entire thoracic contour and make the result as aesthetic and masculine as possible.

In some cases not only the gland has grown but also the areola and even the skin has sagged significantly. This is usually due to a massive weight loss or weight gain, when this happens, we must remove all the excess skin, gland and fat so the intervention will consist of a triple intervention: in a first step we will remove this excess skin, gland and fat by making an incision under the pectoral muscle; in a second step we will perform a vibramatic liposuction to regularise the thoracic contour and give the thorax the most masculine shape possible; and lastly we will perform a nipple transplant adapted in shape, diameter and situation to the man and the person we are treating.

Gynecomasty, although in the majority of cases it consists of very simple interventions aimed at removing only the excess gland, must be individualised to give each case the most appropriate surgical procedure.

Yes, in some cases where the gynaecomasty is not very large, the procedure can be performed under local anaesthesia and on an outpatient basis, which greatly reduces recovery times and financial costs. Even medium-sized gynecomasty can be performed under local anaesthesia. Each individual case must be assessed on a case-by-case basis.

Yes, whenever a breast or a part of it is removed, it must be analysed pathologically, it is obligatory. The breast is an organ that even in men can produce malignant degeneration in the long term and should therefore always be analysed. Most of the time the pathological report is normal.

Drainages are no longer routinely necessary. Several studies published many years ago have shown that drainages do not reduce the risk of haematoma or seroma, but increase the risk of infection by communicating the exterior with the surgical site. The scientific basis states that they should only be used in very particular cases because they increase the risk of infection and do not benefit the patient, despite this, there are centres where they continue to be used habitually.

Drainages, if they have to be used, should be removed a few days after surgery, because if a seroma forms, it remains for weeks and the presence of the drainage does not facilitate its evolution. Nor does it prevent the formation of haematomas; on the contrary, they obstruct the duct.

For these reasons, the evolution of science dictates that drainages should not be used routinely, which unfortunately does not prevent the existence of centres that, due to their lack of scientific update, continue to use them.

In a simple gynecomasty, also called subcutaneous or periareolar, the stitches are all resorbable so they are not removed. In the case of gynecomasty with grafting, it is necessary to remove the stitches from the areola but the rest of the stitches that are placed are also resorbable.
