Single-Anastomosis Duodeno-Ileal Bypass with Sleeve Gastrectomy (SADI-S) Procedure in Madrid

SADI-S (Single Anastomosis Duodeno-Ileal Bypass with Sleeve) bariatric surgery has established itself as one of the most effective and long-lasting techniques for the treatment of severe obesity and type 2 diabetes. At Clínica 3 de Mayo, Dr. Jesús Lago, a specialist in metabolic and obesity surgery, performs this advanced intervention with a personalized, safe approach focused on the patient’s long-term well-being.

What is SADI-S surgery?

SADI-S is a modern evolution of the classic duodenal bypass or duodenal switch, with a simplified technique that reduces risks and improves nutritional absorption.
It consists of two main steps:

  • Gastric sleeve (vertical gastrectomy): the stomach is reduced in size to limit the amount of food the patient can eat.
  • Single intestinal bypass: the duodenum is connected to the end of the small intestine, which decreases the absorption of fat and calories.

Thanks to this combination, the patient eats less and absorbs less, achieving a more profound and lasting weight loss than with other techniques.

Advantages of SADI-S over other bariatric surgeries

  • 🔹 Greater long-term weight loss: between 80% and 90% of excess weight during the first 18-24 months.
  • 🔹 Effective control of type 2 diabetes: in most cases, glucose levels normalize without medication.
  • 🔹 Lower risk of malnutrition compared to traditional duodenal switch.
  • 🔹 Less reflux compared to gastric sleeve alone.
  • 🔹 Permanent surgery, with a low percentage of weight regain.

Dr. Jesús Lago has extensive experience in mixed techniques (restrictive and malabsorptive), which guarantees predictable results and specialized follow-up.

Who is a candidate for SADI-S surgery?

SADI-S is indicated for patients with:

  • BMI greater than 40, or greater than 35 with associated diseases (diabetes, hypertension, sleep apnea, fatty liver, etc.).
  • Patients with severe obesity who have not achieved results with diets, drugs, or endoscopic treatments.
  • People with metabolic syndrome or insulin resistance.

Each patient is comprehensively evaluated at the Clínica 3 de Mayo, with nutritional, endocrinological, and psychological studies prior to the procedure.

How SADI-S surgery is performed

The procedure is performed laparoscopically or robotically, with small incisions and a quick recovery. The procedure lasts between 2 and 3 hours and requires an average hospital stay of 2 to 3 days.

Dr. Lago’s team applies minimally invasive surgical techniques and accelerated recovery protocols, which reduce pain, improve comfort, and shorten the time it takes to return to normal life.

Expected results of the SADI-S

Patients experience progressive and sustained weight loss:

  • At 6 months: 50-60% of excess weight.
  • At 12 months: 70-80%.
  • At 24 months: up to 90%, depending on dietary compliance.

In addition to weight loss, there is an improvement or resolution of associated diseases:

  • Type 2 diabetes (up to 90% of cases).
  • High blood pressure.
  • Hypercholesterolemia.
  • Sleep apnea.
  • Metabolic syndrome.

Recovery and life after SADI-S

Recovery is gradual and carefully monitored. During the first few weeks, patients follow a liquid diet, then a semi-soft diet, until they gradually return to a normal diet.

Follow-up includes laboratory tests, adjustment of vitamin supplements, and psychological support. In the medium term, patients report a better quality of life, increased energy, and metabolic stability.

How much weight can be lost with SADI-S?

On average, 80-90% of excess body weight is lost. This means that a person with 60 kg of excess weight can lose between 45 and 54 kg. The result depends on dietary compliance and regular check-ups with the medical and nutritional team.

Is the surgery reversible?

No, SADI-S is not reversible. However, by preserving part of the functional bowel and making only one anastomosis (one connection), nutrient absorption is better than in the classic bypass or the original duodenal switch. This allows a more stable balance between weight loss and nutritional maintenance.

What are the risks of this surgery?

Like all major surgery, it may involve anesthetic risks, bleeding or infection, although its complication rate is very low when performed by an experienced team. In the long term, some patients may require chronic vitamin supplements, especially iron, calcium, vitamin D and B12. At Mayo Clinic 3, the follow-up protocol allows early detection and correction of any nutritional alteration.

Is it better than traditional gastric bypass?

The SADI-S offers superior weight loss and better metabolic control in patients with severe obesity or advanced type 2 diabetes. In addition, by having only one anastomosis (one intestinal junction), it reduces the risk of leaks and complications with respect to the classic bypass. However, the choice of technique depends on the patient’s individual profile, which is determined after a complete medical evaluation.

How long does it take to get back to normal life?

Most patients can resume daily activities after 10-14 days. Moderate physical exercise is recommended from the first month, and return to work is usually possible within 3 to 4 weeks after surgery.

Is the result long-lasting?

Yes, SADI-S is one of the most durable techniques in terms of sustained weight loss. At 5 years, more than 80% of patients retain at least 70% of the weight lost, proving its long-term effectiveness when combined with a healthy lifestyle.
