Bariatric Rescue Surgery

Safe and effective solutions when the first surgery was unsuccessful

In some cases, people who have undergone bariatric surgery (such as gastric bypass, gastric sleeve, or gastric banding) do not achieve the expected results or experience complications. For these patients, there is an option: bariatric rescue surgery, also called revision or secondary surgery.

At Clínica 3 de Mayo, we are specialists in advanced bariatric surgery and comprehensive obesity treatments. We offer an expert and safe approach for patients who have undergone previous surgeries and need a review, correction, or a new surgical strategy.

What is bariatric rescue surgery?

Rescue surgery involves modifying, correcting, or replacing a previous bariatric procedure that has not achieved its goal (adequate weight loss, resolution of comorbidities, or patient stability).
This type of surgery requires a high degree of specialization, as it involves operating on a previously operated digestive tract, with possible adhesions, anatomical changes, or nutritional deficiencies.

When is revision surgery recommended?

The main situations in which bariatric rescue surgery may be indicated are:

  • Weight regain after initial satisfactory weight loss
  • Insufficient or no weight loss
  • Anatomical complications (fistulas, hernias, severe reflux, stenosis)
  • Malabsorption or malnutrition problems
  • Serious side effects (chronic vomiting, persistent abdominal pain)
  • Gastric band malfunction
  • Patient desire to change the type of technique (e.g., from band to bypass)

Types of bariatric rescue surgery

Treatment will depend on the type of previous surgery and the patient’s current condition. Dr. Jesús Lago performs a complete evaluation before recommending one of the following options:

Conversion from gastric band to gastric bypass or sleeve gastrectomy

The gastric band may cause complications or intolerance and may need to be removed and replaced by a procedure such as Roux-en-Y gastric bypass or vertical sleeve gastrectomy (gastric sleeve).

Gastric bypass revision

The bypass can be reconfigured if there has been dilation of the gastric reservoir or intestinal loop, or if there are mechanical complications. Sometimes a distal bypass is performed to increase malabsorption or place a band over the bypass.

Gastric sleeve revision

When a gastric sleeve loses its effectiveness (due to dilation), it can be converted into a gastric bypass or a duodenal switch, which are more effective techniques for weight loss and metabolic control.

Correction of anatomical complications

This includes fistulas, internal hernias, stenosis, or functional disorders. It requires specific experience in revision digestive surgery and multidisciplinary follow-up.

Personalized assessment before deciding

Dr. Jesús Lago conducts a comprehensive study with imaging tests (endoscopy, digestive CT scan, intestinal transit), complete blood work, and nutritional and psychological evaluation.
This comprehensive approach allows us to make the best surgical decision in each case, considering the risks, benefits, and expectations of the patient.

What results can I expect?

Rescue surgery can enable:

  • Resumption of effective and controlled weight loss
  • Improvement or resolution of associated conditions (type 2 diabetes, hypertension, sleep apnea, etc.)
  • Correction of digestive discomfort or complications
  • Improvement in quality of life and self-esteem

The outcome depends on multiple factors: type of previous surgery, patient age, level of commitment to treatment, and postoperative follow-up.

Do you have questions about your previous surgery? Schedule a review.

If you had surgery years ago and are dissatisfied with the results or experience discomfort, you are not alone. Many patients require a second approach with a new strategy.

Yes, it is more technically demanding and carries a higher surgical risk, but when performed in experienced centers such as Dr. Lago’s, the results are usually very good. Postoperative follow-up is key.

Yes. Many patients lose weight gradually and safely, especially if they follow an appropriate nutritional, psychological, and physical activity plan after the procedure.

It depends on your health, any complications you have, and how your weight is progressing. In general, it is considered after 12 months, although this may vary.

It depends on the autonomous community and the clinical case. In many cases, it is performed in private centers because they are complex surgeries or do not meet public criteria.

Digestive endoscopy, abdominal imaging tests, complete blood work, psychological and nutritional assessment, among others. Dr. Lago coordinates everything from his office.
